HIV and Hepatitis C (HCV) Screening and Health Care Access.
HEPPAC partners with primary care clinics in our community to provide a path to health care access for recently diagnosed and out of care HIV and Hepatitis C individuals.

HEPPACs screening and linkage activities include the following:

• HIV and HCV care and treatment appointment scheduling 
• Patient navigation/advocacy within medical systems
• Linkage to health care coverage
• Client transportation to scheduled appointments
• Referral services
• Basic needs services.
